DECLARATION

consent to the collection, storage and processing of personal data

by an association designated to carry out the public benefit activity “Campaigns and Activism for Animals in Industry” (“CAAI”)

 

This declaration is made in my capacity as a data subject and under the terms of Regulation (EU) 2016/679,

DECLARE

Consent to provide the administrator of the NGO “CAAI”, with the UIC: 177163178 and with the registered office and address of management. “”, Sofia, ul. “Name, surname, e-mail address and work position, subject to the provisions of the General Data Protection Regulation (Regulation (EU) 2016/679).

I declare that if my personal data changes during the processing period, I will notify the administrator “CAAI” in writing.

I am aware that:

  1. The purpose of the processing of personal data is the present survey, namely: to investigate
    the current situation in educational institutions with regard to animal experimentation and
    the opinion of students/faculty members about it.
  2. The personal data provided by me will be processed and stored solely in connection with the purposes of this survey.
  3. Access to my personal data will be granted to the controller “CAAI”.
  4. My personal data will be kept for the minimum period absolutely necessary to achieve the purposes of the processing. After the expiry of the retention period, my personal data will be duly destroyed.
  5. Failure to provide personal data may result in inability to participate in this survey.
  6. I have the right to withdraw my consent at any time. Withdrawal of consent does not affect the lawfulness of processing based on consent given by me before its withdrawal.

I am aware that as a data subject and in connection with the consent I have given for the processing of personal data, I have the following rights:

  1. access to their personal data;
  2. request the rectification of my personal data that is inaccurate or incomplete;
  3. request erasure (right to be forgotten) of personal data relating to me where:
  • my personal data is no longer necessary for the purposes for which it was collected or otherwise processed;
  • withdraws its consent and has no other legal basis for the processing;
  • object to the processing and there are no legitimate grounds for the processing which override;
  • my personal data has been unlawfully processed;
  • my personal data must be erased in order to comply with a legal obligation under Union or Member State law to which the controller is subject.
  1. require restriction of processing where:
  • the personal data are not accurate, in which case the restriction is for a period during which the controller must verify the accuracy of the personal data;
  • the processing of the personal data is unlawful, but I do not wish it to be deleted, only to restrict its use;
  • the controller no longer needs the personal data for the purposes of the processing, but the data are necessary for the establishment, exercise or defence of legal claims;
  • I have objected to processing – pending verification whether the controller’s legitimate grounds override my interests as a data subject.
  1. my right to data portability – the right to receive the personal data I have provided in a structured, commonly used and machine-readable format where processing is carried out by automated means.

I am aware of my right not to be the subject of a decision based solely on automated processing, including profiling (‘profiling’ means any form of automated processing of personal data consisting in the use of personal data for the assessment of certain personal aspects relating to an individual), which produces legal effects concerning me or similarly significantly affects me, unless the decision is necessary for entering into, or the performance of, a contract between me and a controller or is based on the explicit consent of the controller or is subject to the consent of the controller.

I am aware of my right to complain to the Data Protection Commission.
